Medical Certificate form
To be completed by any medical practitioner consequent to a basic medical examination. Click here to download a Medical Certificate form.
Alternatively, a Certificate of Health Examination will be accepted.

Radiological Form To be completed by any radiologist consequent to a chest x-ray, attached hereto for ease of reference. Click here to download a Radiological Report form.
Alternatively, a Certificate of Health Examination will be accepted.

Original Police Clearance Certificate

If renewing the current visa in South Africa, only the South African Police Clearance Certificate is required and this will be done at the VFS office on the day of submission.

Three (3) months bank statements stamped by the bank (please obtain these closer to the time of submission)

EXPIRY OF DOCUMENTS: Medical, Radiological and Police Clearance certificates are only valid for six months from their date of issuance. All other documents expire after three calendar months of their issuance. However, please note, that Birth, Marriage, Death and Divorce Certificates / Decrees have no expiry date.

TRANSLATIONS OF FOREIGN LANGUAGE DOCUMENTS: All documents must be original. Any and all documentation not in the English language must be accompanied by sworn English translations thereof. We can assist with these where required.
